What This Role Is

The Patient Care Coordinator (PCC-I) is the operational heartbeat of a Benchmark Physical Therapy clinic. You're not just answering phones — you're converting referrals into booked evaluations, ensuring patients are greeted warmly, processing payments, verifying insurance, and keeping the schedule running without gaps. It's a multifaceted entry-level position that demands someone equally comfortable behind a computer and in front of a patient.

About Benchmark Physical Therapy

Benchmark Physical Therapy is a brand partner within the Upstream Rehabilitation network, a growing physical therapy organization. Clinical teams here focus on delivering meaningful patient outcomes within a workplace culture that Upstream promotes publicly — known under the #upstreamfamily identity on Instagram and LinkedIn. This Jacksonville location is part of that expanding footprint, and the Patient Care Coordinator sits at the center of how each clinic operates day to day.

What You'll Do Day-to-Day

  • Greet every patient and visitor who enters the clinic with a friendly, professional manner

  • Verify patient insurance coverage ahead of each visit
  • Collect patient payments at the time of service and manage end-of-day deposits
  • Convert new referrals into scheduled evaluation appointments
  • Place outbound welcome calls to new and upcoming patients
  • Monitor appointment arrival rates and proactively address scheduling gaps
  • Maintain an efficient, organized front-office schedule in coordination with the Clinic Director
  • Route clinical and administrative documents to the appropriate teams
  • Complete non-clinical documentation and compliance-related paperwork accurately
  • Track clinic supply inventory and flag replenishment needs

  • Complete quarterly compliance training as required

What You Need to Qualify

  • High School Diploma or GED equivalent
  • Clear, effective communication skills — written and verbal — for working with clinical staff and leadership
  • Demonstrated ability to multitask across multiple computer programs simultaneously
  • Strong attention to detail and organizational skills; able to meet deadlines without close supervision
  • Solid time management skills and comfort working as part of a team
  • Able to sit at a workstation for the majority of the shift, with sustained computer use throughout the day
  • Able to occasionally lift, carry, push, or pull up to 20 lbs

Skills Breakdown: Must-Have vs. Helpful Context

This is an explicitly entry-level designation (PCC-I), so the hard requirements are intentionally minimal — a high school diploma and the soft skills to handle a busy front office. The non-negotiables are strong communication, multitasking across computer systems, and the discipline to work independently without micromanagement. There are no stated nice-to-haves in this posting. Candidates with prior customer service, medical reception, or administrative backgrounds will find the day-one learning curve lower, since the role includes hands-on training for clinic software (Raintree) from the start.
