What This Role Does

Counterpart Health is hiring a Provider Success Manager to own relationships with primary care practices across the DFW region. The job sits at the intersection of clinical consulting and account management: you will onboard practices onto Counterpart Assistant, embed the tool into their daily clinical workflows, and use performance data to help them hit value-based care targets. This is a hands-on, relationship-intensive position requiring equal parts healthcare operations fluency and the ability to influence busy clinicians and their staff.

About Counterpart Health and Clover Health

Counterpart Health originated as an internal clinical-tools initiative within Clover Health and has grown into a dedicated product company. Its flagship platform, Counterpart Assistant, surfaces AI-driven clinical prompts for primary care physicians during live patient visits—designed to catch chronic conditions earlier and support better long-term management. Thousands of practitioners now use it in daily care delivery. Counterpart Health operates as a Clover Health subsidiary; Clover is a publicly traded Medicare Advantage insurer with a technology-forward approach to lowering total cost of care.

What You Will Be Doing

  • Present and demonstrate Counterpart Assistant to both prospective and current partners, connecting the platform's capabilities to each practice's specific value-based care objectives
  • Develop and execute structured onboarding and training programs for practice staff—clinical and non-clinical alike—to build lasting platform proficiency
  • Partner with the go-to-market team to refine provider recruitment playbooks and document effective in-office workflow integrations
  • Analyze practice-level performance dashboards to proactively surface utilization trends, emerging risks, and optimization opportunities
  • Design and implement workflow interventions that make Counterpart Assistant a natural part of each practice's daily clinical routine
  • Serve as the voice of the provider within Counterpart Health—relaying field feedback to product, sales, and operations teams to inform platform and support model improvements
  • Own key engagement metrics across your portfolio and report progress against defined success benchmarks

  • 5–7+ years in provider engagement, healthcare account management, or value-based care consulting with a measurable track record of driving adoption and retention
  • Demonstrated ability to build collaborative, change-driving relationships with physicians and practice staff
  • In-depth knowledge of primary care or ambulatory care clinical operations and workflows
  • Hands-on experience with EHR systems, ideally within a primary care environment
  • Solid understanding of medical billing, reimbursement structures, Medicare Advantage, and risk-based payment models
  • Strong project management skills—comfortable juggling multiple practice relationships and competing priorities simultaneously
  • Ability to translate operational and performance data into clear, actionable scorecards and recommendations for practice leaders
  • Must reside in the Dallas-Fort Worth, Texas metro area
  • Willingness to travel up to 75% of the time (up to five days per week) to visit provider practice sites

Preferred (But Not Required)

  • Familiarity with value-based care models, population health management, or formal quality improvement methodologies
  • Direct experience deploying or implementing VBC-focused technology products inside primary care settings
  • Working knowledge of Medicare insurance products, annual wellness visit workflows, accurate chronic-condition coding, HEDIS measures, and CMS Star ratings

Skills That Matter Most Here

The hard floor is clinical account management: if you have not navigated a busy PCP office and convinced skeptical clinicians to change how they document or review patients, this role will be a steep climb. EHR experience, Medicare Advantage fluency, and comfort with performance data are all non-negotiable. The preferred skills—HEDIS, Stars, population health—sharpen your competitive edge but will not substitute for core operational experience. Seniority sits at mid-to-senior individual contributor: broad autonomy and independent judgment are expected, not prescribed workflows.
