What You'll Do Day-to-Day
- Travel to clients across your assigned geographic territory in the Wyomissing area to conduct in-home behavioral health nursing visits
- Carry out individualized care plans, including administering prescribed medications, performing wound care, and executing ordered treatments and procedures
- Observe and evaluate each client's condition at every visit; report changes promptly to the supervising Clinical or Client Services Manager
- Review physician orders, act on them in a timely manner, and maintain accurate documentation of all order follow-up
- Conduct formal patient assessments as clinically indicated
- Perform case management and coordinate care across the broader clinical team
- Record all nursing observations, interventions, and evaluations electronically using a touch-screen tablet platform
Must-Have Qualifications
- Active Registered Nurse (RN) license issued by the Commonwealth of Pennsylvania
- Minimum one year of recent, verifiable clinical nursing experience in a medical or surgical setting
- Nursing degree or diploma from an accredited, approved program (transcript or diploma required as proof)
- Ability to work independently and manage a self-directed schedule without on-site supervision
